Beloved husband, father, grandfather, son and brother Stephen R. Usiak III of Las Vegas, NV, passed away at home surrounded by family Sunday, Dec. 30, 2012. He was born in International Falls, MN, Oct. 7, 1953, to Stephen R. Usiak Jr. of Elizabethtown, KY, and the late Gloria Usiak.
Survived by his wife of 27 years, Rosa, son, Mark (Julie Ann), daughter, Danielle, grandchildren, Caden, Jagger, and Stephen Usiak, all of Las Vegas, NV; brothers, Kim Usiak of Coral Gables, FL, Mark Usiak of Chicago, IL, and Tony Usiak of O’Fallon, MO; sisters, Holly (Bob) Heinold of Franklin, TN, and Michelle (Charles) Burris of Westerville, OH; several nieces, nephews and many other family members and friends.
A memorial service will be conducted at 2 p.m. Saturday, Jan. 12, 2013, at Palm Mortuary, 1325 North Main St., Las Vegas, NV 89101.
In lieu of flowers, his family would prefer a donation be made to St. Jude Children’s Research Hospital or the American Cancer Society.
